Check the Marching Cavaliers tab for the "Medication Form." This form is to be filled out, and submitted on Sunday, prior to boarding the bus for band camp. Medications must be signed over to the Directors by a parent or guardian.
All medications must be in their own prescription bottles, with clear labels for the student's name, name of medication, dosage, schedule for dosage, use, and prescribing physician.
All medication bottles must be in a plastic zip-lock bag with the students name clearly labeled on the bag.
